Goal: To provide safe administration of medications to students during school hours.
*All medications must be brought in and picked up by parent/guardian.

will be administered by the school nurse or school personnel.
2. Medication must be in the recent, original container with student's name, date,
doctor's name, name of medication, dosage to be administered.
Over the counter medications such as Tylenol, Ibuprofen, etc. must be in the original box or container when brought to nurses's office. Please remember it needs to be age appropriate.
3. Parent/guardian must bring in the medication to the nurse and sign a medication authorization form before the nurse/designated personnel can administer the medications.
4. Medications are to be administered at school only if they cannot be administered
at home or if specified by a licensed Doctor.
5. Medications will be kept in a secured place in the nurses office. Medications such as epinephrine/inhaler/diabetic supplies, should not be kept in class or with student unless ordered by a licensed Doctor and the parent/guardian agrees and signs a self carry medication authorization form with the school nurse first.
6. Each case where a physician states a student can carry own medications(asthma
inhalers) while in school shall be reviewed for safety and decision making with
doctor's order, nurse, parent and school administration.
7. Cough drops, eye/ear drops, over the counter/prescribed ointments need to be brought to the nurse by the parent/guardian and a medication authorization form needs to be signed by the parent/guardian if it requires administration during school hours.
Forms for medication administration are available in the nursing office
at each campus.
